Health history questionnaires will need to be completed online prior to your initial naturopathic consultation. New patients will receive a link to set up their health records account after their first visit is scheduled. Your practitioner uses Charm PHR portal to keep all patient information confidential. Please complete your health history 48-hours prior to your first appointment.
